End of life decisions for patients
As a Catholic Health Care provider, Little Company of Mary Hospital and Health Care Centers follows the guidelines established in the "Ethical and Religious Directives for Catholic Health Care Services" regarding End of Life Decisions. We are committed to guiding our patients and their family members through these difficult times with compassion and sensitivity.
The chaplains in our Pastoral Care Department can help you understand Advance Directive forms and help complete them with you. Please feel free to contact us at 708.229.5480 for any questions or concerns regarding these issues.
Advance Directives are written instructions under state law relating to an individual's health care when he or she is incapacitated. Advance Directives take two forms:
- Living Will
- Durable Power of Attorney for Health Care
The purpose of the Living Will and the Durable Power of Attorney for Health Care is to enable you to state your wishes clearly in writing and to appoint someone to make choices for you if you become unable to make decisions. In order to help you gain a complete understanding of Advance Directives, the Pastoral Care Department offers six educational programs per year to help you reach an informed decision. We are there to answer any questions you may have and to help you complete your Durable Power of Attorney for Health Care.
Please call 708.229.5480 for dates and times.
