Skilled At Home Care

For more than 25 years, Home Based Services has provided intermittent care by a team of specially trained health care professionals, based on the clinical needs of the patient. Home Care bridges the gap between hospital or nursing facility and where the patient prefers to be - their own home. The goal is for the patient to attain independence and remain in the setting they find most comfortable. Many technological medical advances allow care previously available only in an institution to be delivered in the patient's home.

 

Skilled Nursing Care including:

Comprehensive assessment, ongoing monitoring, skilled interventions such as wound and catheter care, health care education, IV therapy, mother/baby visits.

 

Physical Therapy including:

Strengthening, gait and stair training, transfer training, post-joint replacement exercises, home safety.

 

Occupational Therapy including:

Use of adaptive devices, relearning of activities of daily living (ADLs)

 

Speech Therapy including:

Speech and swallowing exercises.

 

Medical Social Services including:

Financial and emotional counseling, information regarding community resources.

 

Home Health Aides including:

Assistance with ADLs and personal care.

 

Specialty Programs including:

Congestive heart failure and diabetic pathways, anticoagulation therapy program.

 

Other Services including:

In-home protime tests, medical supplies, specimen collection, dietary teaching, arrangements for medical equipment.

 

All of the above services are provided to you by Home Based Services' team of licensed and certified health care professionals. This team consists of: Registered Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, Licensed Clinical Medical Social Workers, and Home Health Aides. Also on staff at Home Based Services are specially certified clinicians with expert knowledge and skill in specific areas. These include a Certified Wound Ostomy Continence Nurse (CWOCN), three Certified Hospice Palliative Care Nurses (CHPN), and two Cancer Chemotherapy Providers, a Certified Hospice and Palliative Care Nursing Assistant (CHPNA), and a Certified Hospice and Palliative Care Administrator (CHPCA).
