Medical Care for Homebound Patients
Mobile Medical Care is a community outreach program offering primary care provider services directly in patients' homes. Our Nurse Practitioners, working in collaboration with our Medical Director, make home visits ensuring that homebound patients have access to health care on a regular basis. For the past 20 years, the program has been bringing medical care to the frail, weak, homebound individual unable to travel to a primary care provider's office. With Mobile Medical personnel, patients receive their necessary services in the comfort and safety of their home surroundings. Once seen for the initial vsit, the Nurse Practitioner will set up future visits based on each person's individual need. The Medical Director consults with the Nurse Practitioners and may see patients on occasion as needed.
The team offers a service that is vital to many who may not otherwise have access to medical care, because of health problems that limit their mobility. The team makes more than 1,100 visits a year and received the 2009 Sister Nancy Boyle Award for their devotion and compassion in caring for the sick. Mobile Medical Care upholds the mission of the Sisters of Little Company of Mary, providing the personal care for which Little Company of Mary is known.
