A representative of Little Company's Case Management Department may contact you during your stay to discuss your post-hospital plans. Many patients need medical care after discharge. While you, your family and your physician make the final decision concerning your post-hospital care, the social worker or care coordinator can provide you with the information you need to arrange special services for you provided that medical necessity criteria are met. These services include: home health care, skilled nursing care, rehabilitative care, adult day service and medical equipment.
