Birth Certificates


Birth certificates can be retrieved from the Evergreen Park Village Hall. We do not keep birth certificates at Little Company.

You can request a birth certificate by mail or in person at the Village Hall.

The cost is $10 for the first copy, $2 for each additional.

To send a request by mail: send a request with a name, date of birth, daytime phone, copy of picture ID, a self-addressed stamped envelope, and the payment in a money order or certified check. Send your request to:
