Advance Directives are written instructions under state law relating to an
individual's health care when he or she is incapacitated. Advance Directives
take two forms:
- Living Will
- Durable Power of Attorney for Health Care
We believe that life is a precious gift, one that we have an obligation to
enjoy and protect. An essential part of life is the quality of life that we live
and value. During times of prolonged illness, this quality may change and become
unacceptable to you. The best way for you to retain control is to let your
wishes be known by recording your preferences in advance. It is also very
important that you talk to your loved ones and those close to you about your
personal wishes regarding health care. The purpose of the Living Will and the
Durable Power of Attorney for Health Care is to enable you to state your wishes
clearly in writing and to appoint someone to make choices for you if you become
unable to make decisions.
Where Should I Keep My Advance Directive? We recommend
that you keep the original form, give a copy to your physician and a copy to
your agent. A copy will also be placed in your current medical record.
