Requires a level of knowledge normally acquired through the successful completion of an Associate Degree Nursing Program. Maintains a current license as a registered nurse in the state of Illinois and possesses a valid driver's license. Will be required to complete CPR certification and maintain recertification. Requires one to two years of progressively more responsible work experience as a licensed nurse within a home care, hospice, medical surgical or oncology setting. Conducts the initial physical assessment of patients and their family. Coordinates the preparation of patient care plans. Conducts periodic home visits to the patients based on the patient's level of need. Coordinates and oversees the delivery of service provided by Nurse Assistant. Prepares patient care notes and ensures medical charts are up to date. Contacts attending physicians with requests for orders or changes in orders. Acts as an on-call Homecare Nurse. Conducts instructional and informational programs for Homecare staff. Attends and participates in staff meetings. Responds to patient and family requests in emergency, and crisis visit situations. Coordinates the delivery of occupational speech and physical therapies to Homecare patients. |
