Please be advised that all physicians who provide services as part of the bariatric program will bill you separately. Those physicians include the bariatric surgeon, the assistant surgeon, anesthesiologist, pulmonologist, pathologist, psychologist, and perhaps additional physicians who may be involved in providing you with quality medical care. We understand that each physician involved in the bariatric program has standard self-pay rates, so that you can be given a 'package rate' for your bariatric surgery. The Program Coordinator will make contact with each physician to notify them of any intentions of self-pay for bariatric surgery, so that, if necessary, you will be billed the appropriate self-pay rate for their services.
Self-pay requirements provide that you make your payment to cover the hospital services for bariatric surgery by certified check, money order, or credit card on or before the date of your scheduled pre-admission testing. If you do not have the appropriate form of payment with you on the date of testing, or if you have not made payment prior to that date, we cannot provide service to you.
