Registered Nurse - Intake (Days)

Responsible for the discharge planning activities for potential patients of the Home Based Services Department.  Receives patient referrals from area hospitals, nursing homes and social workers.  Submits referred patient data for assessment and determination of program eligibility through knowledge of third party reimbursement regulations.  Provides patients and family members with information about the Home Care services, to include instructions regarding treatments, equipment use and/or resources for procurement of needed supplies.  Contacts physicians and obtains patient orders.  Coordinates and communicates new patient information to the Home Care staff and participates in home visits, as necessary.

Current IL Registered Nurse licensure and 2 - 3 years progressively more responsible work experience preferably within a home care, hospice or medical/surgical setting are required.  Strong interpersonal skills necessary to provide social and psychological support to patients and their families while eliciting relevant medical and insurance information.
