Master's degree in Social Work and Licensed Clinical Social Worker required.  Requires demonstrated knowledge of bereavement concepts - ability to relate these concepts to personal experience.  Requires knowledge of community resources.  Must have basic computer skills.  Must have an automobile for work and a valid drivers license.  Requires 1 year experience in home health or hospice preferred.  Able to address issues related to erminal illness, death and dying, to loss, grief and berevement, from the time of the patient's admission to hospice, throughout the course of care and specific time frames following the death of the patient.
